EFT ডেবিট ফর্ম EFT Debit Form ফর্মটি ডাউনলোড করে পূরণ করুন, তারপর হেড অফিসে জমা দিন। Download the form, fill it in, then submit it to the Head Office.

নিচের ধাপগুলো ক্রমানুসারে অনুসরণ করুন। Follow the steps below in order.

    EFT ফর্মটি ডাউনলোড করুন (যদি ফর্মটি না থাকে, তাহলে একটি PDF সরবরাহ করা হবে)।

    ফর্মটি সঠিক ব্যাংক তথ্যসহ মনোযোগ সহকারে পূরণ করুন।

    ফর্ম পূরণে কোনো সহায়তা প্রয়োজন হলে, জনাব মোঃ শাহ আলম (ফোন: ০১৩৩৫১০২৭০৬) এর সাথে যোগাযোগ করুন অথবা নিকটস্থ প্রগতি লাইফ অফিসে যান।

    ফর্ম পূরণ সম্পন্ন হলে এটি প্রগতি লাইফ হেড অফিসে পাঠান যাচাই ও প্রসেসিংয়ের জন্য।

    হেড অফিসের ঠিকানা: প্রগতি লাইফ ইন্স্যুরেন্স লিমিটেড, প্রগতি ইন্স্যুরেন্স ভবন (১১ তলা), ২০-২১, কারওয়ান বাজার, ঢাকা-১২১৫।

    যাচাই সম্পন্ন হলে EFT এর মাধ্যমে স্বয়ংক্রিয়ভাবে প্রিমিয়াম সংগ্রহ করা হবে।

    Download the EFT Form (if you do not have it, a PDF will be provided).

    Fill out the EFT form carefully with accurate banking details.

    If you need help filling out the form, contact Mr. Md. Shah Alam (Phone: 01335102706) or visit your nearest Pragati Life branch office.

    After completing the form, send it to the Pragati Life Head Office for verification and processing.

    Upon successful verification, your premium will be collected automatically through EFT transfer.
